Low-intensity shockwave therapy (Li-ESWT)
Li-ESWT is a non-surgical, session-based method in which low-energy sound waves are applied to penile tissue. The aim is to contribute to the quality of erections by supporting the vascular structure.
In terms of suitability, the most promising group is people with mild to moderate vascular erectile dysfunction who respond partially to oral medication. In advanced vascular insufficiency, in pictures caused by nerve damage and in severe diabetic cases, the expected benefit falls markedly. The number of sessions and the interval between them are planned individually.
Penile PRP (the P-Shot)
PRP is based on preparing and applying platelet-rich plasma obtained from a person’s own blood. Because their own blood is used, the risk of an allergic reaction is low.
The level of scientific evidence in this field is limited, however; the procedure should not be presented as a proven standard treatment. Discussing the state of the evidence and the likely outcome openly at the consultation matters more than the procedure itself.
Who is it not appropriate for?
In the situations below the procedure may not be appropriate, or another treatment takes priority:
- Conditions that need correcting first, such as uncontrolled diabetes or an untreated hormonal disorder
- Cases with advanced vascular insufficiency or a marked venous leak
- Active infection or a skin lesion in the genital area
- Situations made risky by a bleeding disorder or the use of blood thinners (for PRP)
- Cases where someone comes expecting a “guaranteed result” and no agreement is reached in the conversation about expectations
Managing expectations
These procedures do not take the place of medication, or of surgery where surgery is needed. Expecting a lasting result from sessions alone, without addressing the diabetes, blood pressure, cholesterol, smoking and sleep problems that underlie the erectile dysfunction, is not realistic.
The outcome varies from person to person and no method can be guaranteed. At our Denizli clinic these options are raised only after examination and any necessary tests, and only for those found suitable.
Frequently asked questions
Is shockwave therapy painful?
With low-intensity treatment, a mild sensation of pressure is usually described. Sessions are short and a return to daily life afterwards is generally possible.
How many sessions are needed?
The number of sessions and the interval between them vary with the individual and the protocol used; they are planned by the physician after an examination. It would not be right to give a single standard figure.
Does PRP provide a lasting solution?
The level of evidence in this field is limited and no promise of a lasting result can be given. The procedure does not take the place of treating the underlying causes.
Are stem-cell treatments an option?
Stem-cell based treatments for erectile dysfunction are still at the research stage. Presenting them as a routine treatment would not be correct.
